Please fill out, print, sign and return all forms. All completed forms are due by June 15, 2010. Forms may be turned in by placing them in the box in Mr. Robinson's office. Forms can also be mailed to:

2009 Spiritwear!

• This price list includes the new "Band Camp" T-shirt for adults, which can also be worn later (after Band Camp), while attending (not working) Band events. Please note that parent volunteers & chaperones are to wear their blue PCHS Band polo shirts when working football games, while working the Panther Creek Invitational, and during other competitions.
• Note: The full-zip, Fleece Columbia Blue jacket is no longer manufactured, so we can only order from existing stock. the order deadline for these two items has been extended to Tuesday, June 30th. Please mail the checks to:
