2011/2012 Registration Forms
We are using a new online registration process for the 2011-2012 marching band. This new registration process is required for ALL marching band students (incoming/new AND current students) by June 15, 2011.
Please register using the link below, then print and sign your papers as directed:

Please use one of the following options to turn these forms in:
- Bring them with you (on Tuesday, May 17th) to the marching band kick off meeting at PCHS auditorium. Meeting details have been sent out separately.
- Mail them to the following address along with payment:

Please contact kshah_us@yahoo.com for any problems you may experience completing the forms. We will have forms assistance available during the kick off meeting on May 17th.
Note – We are still working to perfect the format of the online documents, so there is no need for concern that they may not look the same as last year's paper documents. We will have them corrected as we make progress on expanding our web-based capabilities. Thank you for your support!
